Be it mankind or manslaughter, the man had to be the center stage shamelessly. From profundity to profanity, men have invaded, interfered, and interjected at will.
In medicine however, they have stumbled. To put it succinctly, the mob mentality of manhood gets a serious beating when it comes to matters of health. Banter apart, serious health hazards rock these testosterone driven Homo sapiens behind the legendary muscularity and masculinity that is witnessed.
Not surprisingly, men falter whenever it comes to heart or heart related issues. They seem completely unable to handle this organ. Politically, financially and emotionally, heart has ruined men.
In medicine it gets even worse. All scientific literatures point to the fact that male gender alone may contribute to the risk of cardiac heart disease. Landmark trials like ONRAGET and TRANSCEND study populations found that females had approximately twenty percent lower risk than males for all major cardiovascular end points including myocardial infarction and a combined endpoint of death, stroke and heart failure hospitalization. Further studies suggest that differences in cardiac heart disease risk within the male gender are linked to inherited variations in sex chromosomes, which may contribute to the importance of family history as a risk factor of cardiac diseases.
Imposed upon a background of genetically determined susceptibility, men again limp ahead of women in cancer, both in incidences and in deaths. A 2008 worldwide glance at estimated incidences, mortality and prevalence of all cancers excluding non melanoma skin cancers show the obvious disparity. Rates are substantially higher in males than in females in major cancers like colon cancer and lung cancer.
Statistics are there to be seen. Based on the GLOBOCAN 2008 estimates, about 12.7 million cancer cases and 7.6 million cancer deaths are estimated to have occurred in 2008; of these, 56% of the cases and 64% of the deaths occurred in the economically developing world. Lung cancer is the leading cancer site in males, comprising 17% of the total new cancer cases and 23% of the total cancer deaths.
Then there's the alcohol factor and its aftermath. So manly and yet so pathetic. Thus from alcohol related liver diseases, to alcohol driven motor vehicle accidents, men traverse the entire path of self annihilation. Even Shakespeare understood the folly when Porter in Macbeth warned about alcohol that ' increases the desire but takes away the performance.' Yes, sexual drive too. Men suffer, going for that extra peg.
This brings me to suicide, for the facts are both intriguing and dangerous. While women contemplate suicide more, men actually pull the trigger more. Reasons? Depression and stress. Are men more depressed and stressed out than women? Possibly not. The danger lies in its manifestations. Call it chivalry or social expectations; depression doesn't get delivered the way it does in women. No hair tearing event, no dam bursting tears, no threatening letters of suicide. Men try to buckle it down and with disastrous consequences. What comes out is stealthy, sinuous, and fatal. Loss of appetite, loss of weight, loss of concentration, or worse, a simple silence. A vacuous facade that is ill fathomed and ill managed.
Today's world of medicine is evidence based. And evidence based medicine clearly points out that men seek doctors far less than women. A trait that adds fuel to the fire. Genetically and socially predisposed, men need more attention than ever. They might shine like a medieval knight but inside is a vulnerability that must not be denied. A firm reminder thus, that when it comes to health and disease, it is best prevented than tackled.
In medicine manliness is not a virtue.
